Screen Name
Your screen is used to identify your postings and contributions to The Science Advisory Board without giving out your real name or contact information. It must be at least 4 characters long and cannot exceed 50 characters.


Password/Verify Password
Your password is used in combination with your user name to log in to The Science Advisory Board. It must be at least 4 characters long and cannot exceed 20 characters.


Password Retrieval Question/Answer
If at any time you forget/misplace your password, you will be able to retrieve your password online if you can supply your user name and answer your password retrieval question correctly with the password retrieval answer.

Choose a question that only you know the answer to and that has nothing to do with your password. For example:
Bad: Who discovered the Small Pox vaccine?
Good: What is my mother's maiden name?

Both the question and the answer are required and can be at most 50 characters each.


First/Last Name
The first and last name are required and can be at most 25 characters each. These fields will not be visible to other members of The Science Advisory Board -- other members will see your screen name.


Email
The email address is required and must be in the form someOne@someDomain.xxx. It cannot exceed 100 characters. Your email address is private, and other members of The Science Advisory Board will not be able to view this field.


Phone
The daytime telephone is required. Please include country and area code. Other members of The Science Advisory Board will not be able to view your daytime telephone.


Institution/Company/Organization
The name of your institution/company/organization is required. This field is private and will not be visible to other members of The Science Advisory Board.


Institution/Company/Organization Description
The best description of your institution/company/organization is required. If at all possible, try to select an option from the list provided. If you cannot describe your company with the options provided, you may select "Other, Please Specify" from the list and then must fill in the adjacent space provided.


Job Position
The best description of your current job position is required. If at all possible, try to select an option from the list provided. If you cannot describe your current job position with the options provided, you may select "Other, Please Specify" from the list and then must fill in the adjacent space provided.


Research Expertise
You must select at least one research area of expertise. If "Other, Please Specify" is selected, you must also fill in the adjacent space to specify your other research area of expertise.


Research Tools/Techniques
You must select at least one research tool or technique. If "Other, Please Specify" is selected, you must also fill in the adjacent space to specify your other research tool or technique.


Clinical Expertise
You must select at least one clinical area of expertise. If "Other, Please Specify" is selected, you must also fill in the adjacent space to specify your other clinical area of expertise.
